Volunteer Application

Any adult that wishes to assist the team in any capacity during the practices or games must complete and submit a Volunteer Application before they can assist with the kids. You can submit this online by going to "My Account" and then selecting "Volunteer." Alternatively, you can print and submit this physical Volunteer Application. There are no exceptions. Your information is kept in complete confidence. Please follow the instructions for submission.

Medical Release Form

Every child must have a Medical Release Form completed before the season begins. You can submit the information online during the registration process or you can print out this physical Medical Release Form. Please return the completed paper form to the Manager or Coach.

Pitching Logs are a convient way to help track the pitches thrown by players throughout a game. You can download one here.
